Specialist Area Requirements: To be responsible for the operational and clinical management of the Renal Home Dialysis Unit To be responsible in ensuring the continuity of patient care between hospital, home and within the department. To provide excellent nursing care, utilise specialist knowledge to teach & educate patients, carers, undergoing Peritoneal Dialysis and Haemodialysis at home. To work collaboratively to lead and coordinate the development of home dialysis unit making best use of available resources. To provide effective and robust training programmes for patients/carers/staff To provide an effective and responsive service to patients and carers within the trust and in their homes. Initiate and manage the treatment of renal failure anaemia, working within agreed guidelines. To provide practice development support to renal nurses. To work closely with the matron, care group lead and service manager to ensure the operational and strategic development of the home dialysis service. Participate in the procurement of dialysis consumables and the home delivery services as appropriate.
