Summary

Major Duties Managerial To provide clinical leadership for all health care professionals in the Practice other than General Practitioners. To develop and implement plans to use the diverse workforce to best meet the needs of our patients. To work with the non-clinical management to ensure local and national targets are met. To ensure all clinical staff receive the training and development required to fulfil their roles. To support the Executive Manager in ensuring all clinical services meet CQC standards. With the support of Partners to provide clinical supervision to a diverse range of health care professionals. Oversee the recruitment, training, and supervision of all Clinical Team members. Provide continued professional development opportunities for team members. Conduct regular performance reviews, providing constructive feedback to achieve positive outcomes. Clinical Practice To carry out a full health assessment of patient health care needs. To provide advice, initiate treatment, and refer to another member of the team as appropriate. To provide and maintain high standards. Prescribe products to meet the needs of patients within the practice population. Refer to other primary, community and secondary care members within locally developed pathways. To work in partnership with General Practitioners and other professionals to develop protocols, standards and audit tools to facilitate effective delivery of care. Home visits as appropriate to patient care including Care Homes. Work with others to change policies to improve health and well-being and reduce inequalities. Therapeutic Monitoring Checking compliance with and adherence to appropriate treatments using a holistic patient-centred approach Recognising abnormalities. Identifying the impact of treatment and implementing or recommending changes as appropriate. Risk Assessment: Recognising issues and gathering sufficient information to refer (e.g., drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems) Education, Training and Professional Development: Maintain continued education by attendance of courses and study days as deemed useful or necessary for professional development and develop and maintain a personal learning plan in line with revalidation requirements. Contribute to the education of primary and community care clinicians of all disciplines with special responsibility for nursing staff. Create a learning environment that promotes the concept of life-long learning. Contribute and participate as required in any research projects within the Practice. Role Responsibilities Managerial: Provide clinical leadership for the Practice Develop and sustain a culture of team working across the Practice. Be innovative in developing new ways of working where appropriate and support staff through change. Manage the team performance including regular appraisals. Work with the Executive Manager and wider team to ensure services are provided efficiently and effectively and integrate with the wider business of the practice. Clinical Use high-level decision-making skills. Undertake consultation at an advanced level, using expert knowledge and clinical skills to ensure patients receive high quality care, delivered in a timely manner. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Make direct referrals to secondary care, mental health and social services within locally agreed pathways. Maintain accurate and contemporaneous records using the Practice clinic system. Protect and promote the health and well-being of patients, their relatives and carers. Work collaboratively with stakeholders across the Health and Social Care sectors, and in partnership with patients and/or carers, develop effective, efficient and preventative care plans. Mentor and support other clinicians and undertake clinical peer reviews. Liaise with the Practices rota GP and other GPs as required. Professional Role Maintain an up to date professional registration Monitor the effectiveness of own clinical practice through the quality assurance strategies such as the use of audit and peer review. Maintain own clinical/professional expertise by participating in clinical activities, personal development planning, clinical governance and revalidation. Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained. Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation. Pro-actively promote your role within the Practice. Promote a professional approach at all times by dress and attitude to patients and colleagues. It will be necessary to attend and contribute to various practice meetings as requested. (The only reason for not attending will be annual, study or sick leave). Support the development of pathways with tertiary services to enable the Urgent Care Clinics to become non-GP led.
