Summary

The post holder will be responsible for the delivery of General Practice Nursing to the population of Bury. The focus of the role is both the delivery of evidence-based practice for Patients presenting with a long term condition, and the provision of preventative health care to the Practice population. As an autonomous Practitioner, the Practice Nurse is responsible for the care delivered to the population of Bury, demonstrating critical thinking and skills in clinical decision-making in the management of Patients. They will work collaboratively with the General Practitioner and other Practice Nurses to support the delivery of a healthier Bury through listening and responding to Patients needs. This list of duties and responsibilities, which follows, represents the broad range of tasks, which may be required to be undertaken either routinely or periodically by the post holder. Clinical Manage the Patient experience in a competent and professional manner. Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being. Assess, plan, implement and evaluate individual treatment plans for Patients with a known long-term condition. Proactively identify, diagnose and manage treatment plans for Patients at risk of developing a long-term condition as appropriate. Work with other Health Care Professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and Practice needs. Review medication for therapeutic effectiveness, appropriate to Patient needs and in accordance with evidence-based Practice and national and Practice protocols. Work with Patients in order to support adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support Patients to adopt health promotion strategies that promote Patients to live healthily, and apply principles of self-care. Support and manage health needs of women presenting for family planning and cervical cytology consultations. Recognise, assess and refer Patients presenting with mental health needs in accordance with the NSF for Mental Health. Implement and participate in vaccination and immunisation programmes for both adults and children. Advise, support and administer vaccinations where appropriate for patients travelling abroad. Promote and deliver evidence-based care for Patients presenting with aural conditions. Meet the needs of Patients presenting for opportunistic wound care. Perform investigatory procedures. To undertake phlebotomy. Give contraceptive advice. Offer specialist information/advice in areas such as blood pressure, weight control, smoking cessation, heart conditions. Service Delivery Recognise and work within own competence and professional code of conduct as regulated by the NMC. Produce accurate, contemporaneous and complete records of Patient consultation, consistent with legislation, policies and procedures. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Deliver care according to NSF, NICE guidelines and evidence-based care. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Support and participate in shared learning across the practice and wider organisation. Participate in the performance monitoring review of the team, providing feedback as appropriate. Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance. Work within policies regarding family violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriate. Administration and Professional Responsibilities Participate in the administration and professional responsibilities of the Practice Team Ensure accurate and legible notes of all consultations and treatments are recorded in the Patients notes Ensure appropriate items of service claims are made accurately, reporting any problems to the Practice Manager Ensure accurate completion of all necessary documentation associated with Patient health care and registration with the Practice Ensure collection and maintenance of statistical information required for regular and ad hoc reports Attend and participate in Practice meetings as required Assist in the formulation of Practice philosophy, strategy and policy To assist in seasonal and special projects as requested e.g. flu campaign Ordering and display of health promotion materials Duty of Candour A duty of candour is a commitment to being open and honest with Patients when something goes wrong with their treatment or care of causes, or has the potential to cause harm or distress. The Practice promotes a culture of being open, honest and transparent. Patients are provided with care that is safe, effective and based on best practice. Where any incidents occur that may have the potential to cause harm, the Practice will act in a timely manner, investigating, reflecting, learning and where appropriate to do so, share information to reduce the risk of reoccurrence. The Practice understands that there are two different types of candour which are statutory duty and professional duty. Whitworth Medical Centre is regulated by the CQC under the statutory duty of candour and certain staff at Whitworth Medical Centre will also fall under the professional duty of candour. Safeguarding All staff are responsible for ensuring that they are familiar with and adhere to Whitworth Medical Centres and the ICBs safeguarding procedures and guidelines in conjunctions with the safeguarding Childrens and Adult Board Policies, Procedures and Guidelines. Prevention of Infection All staff have a responsibility to comply with Whitworth Medical Centres policies for personal and patient safety and for the prevention of healthcare associated infections. This includes rigorous and consistent compliance with policies such as prevention measures as hand hygiene, the use of personal protective equipment and safe disposal of sharps.
