Summary

The following are the core responsibilities of the Advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Clinical Responsibilities To use advanced practice skills within consultation to assess, diagnose, advise, treat and refer patients both in Surgery and on Home visits To provide a comprehensive assessment Arranging and interpreting appropriate investigations Develop, implement and embed health promotion and well-being programmes Support the clinical team in the development of individual treatment plans for chronic disease patients Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health Ensure read codes are used effectively Chaperone patients where necessary Prioritise health issues and intervene appropriately Support the team in dealing with clinical emergencies Recognise, assess and refer patients presenting with mental health needs Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.) Support the clinical team with all safeguarding matters, in accordance with local and national policies Deliver opportunistic health promotion where appropriate Professional Responsibilities Practice within the policies and guidelines of the NMC Maintain confidentiality of all patients identifiable information and contacts Maintain and develop clinical knowledge and competence using best practice, evidence, guidelines & supervision Have a clear understanding of the legal issues and professional accountability relating to advance practice roles Prescribers must comply with current legislation for prescribing and be accountable or their practice; it is best practice to maintain the guidelines of the NMC and local ICB Maintain skills and competencies in the use of the EMIS and System One clinical systems Inform the on call GP or line manager of any concerns during session, report significant events according to policy Maintain accurate records of all patient contacts in accordance with NMC guidelines of best practice Support the clinical team with all safeguarding matters, in accordance with local and national policies Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Participate in the administrative and professional responsibilities of the practice team. Accurate and legible notes of all consultations and treatments are recorded in the patients notes. Ensure clinical systems kept up to date, recording and/or amending accurate details. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice. Attend and participate in practice and or PCN meetings as required. Management Responsibilities Effectively manage workload, planning prioritising and co-ordinating workload Work in partnership with other clinicians on duty to maintain effective communication Promote a safe and effective service Adhere to policy and procedures Support and contribute to the future development of the service In addition to the primary responsibilities, the advanced nurse practitioner may be requested to: Contribute to the implementation of an effective training programme for staff and students within the practices Participate in local initiatives to enhance service delivery and patient care Support and participate in shared learning within the practice Continually review clinical practices, responding to national policies and initiatives where appropriate Be aware of and contribute to the planning and implementation of ICB patient pathway initiatives Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA). Maintain professional registration and adhere to revalidation requirements
