Summary


This fixed term post presents an excellent opportunity for an enthusiastic individual to join a dynamic and highly regarded elderly medicine department. Our department hasan excellent track record in service development, transformation, and integrated working across departments and across the organisation. This post will involve working in outpatient clinics providing both rapid access, admission avoidance clinics for the frail population of Hillingdon and general elderly medicine clinics.



This post is for 1 year fixed term from June 2024 to June 2025 due to career break. The current set up of this post is for 2 full days of clinics and SPA worked flexibly so will suit someone looking for less than full time or flexible working. Our Rapid Access Clinics for the Elderly (RACE) run up to 5 times a week providing rapid assessments of community dwelling frail patients referred by their GP and also follow up for patients seen by our service on the wards or in our Acute Frailty Unit supporting ED. General clinics serve a wide range of presentations in over 65’s where a holistic approach to care is required. We have recently expanded our acute frailty provision and provide 7 day a week frailty SDEC services.

The post-holder will be responsible to the Director of Unplanned Care at The Hillingdon Hospital. The role is a 5 PA post – 4 direct clinical care (DCC), 1 supporting professional activities (SPA) as agreed by the medical director, the latter includes service development, audit and personal CPD.Personal CPD must be included as part of annual appraisal which is mandatory and required for the revalidation process.



The post holder will, together with their consultant colleagues and other health care professionals in the Directorate, provide an effective and comprehensive service for older people and support continuing service developments, audit and clinical governance.



The post holder will participate in the on-call emergency medical rota on an approximately 1:18 basis. During the week, the post holder will be on call from home from 1900-0700, and then complete an onsite post take ward round the following morning from 0700-0900. During split weekend on calls (Saturday or Sunday), the post holder will be present on site from 0700-1900 and then on call from home until the following morning at 0700.



The post holder will be encouraged to develop an area of expertise of special interest that he or she may have in Geriatric Medicine in accordance with the service needs of the department The post-holder will have a continuing responsibility for the care of patients in their charge and for the proper functioning of the department. They will also undertake administrative duties associated with the care of patients and the running of the clinical department.

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.
