Summary


We are looking for a dedicated and passionate Assistant Practitioner to join our Pathway 3 service based in Maidstone.

Pathway 3 is part of the Home First / Discharge to Assess Process.
We support discharge to local care home for individuals who require a further period of assessment to determine the long- term plan of care when an acute hospital admission is no longer required.

We work in collaboration with the acute trust Discharge Liaison Teams, Kent County Council Social Services Teams, All Age Continuing Health Care, local community services and organisations including the voluntary
sector.

We work collaboratively to support the assessment of individuals in a timely way to determine long term need.

In this challenging role you will be triaging all referrals received and assessing if they are suitable to the service.

You will be liaising with referrers, requesting any additional documentation prior to hospital discharge. You'll be uploading documents to RIO so
use of Community Services System RIO would be an advantage, however, training will be provided.

Good use of Excel would be an advantage as you will be navigating a complex caseload sheet. You'll also need to be able to demonstrate great people skills as you will be handling lots of enquires and speaking with patients and their relatives and carers.

Additional Working Pattern: 9.5 hrs (across 4 days, fixed working days to be confirmed, plus rotational Saturdays (1 in 4)
