An Advanced Social Work Practitioner is required to join the Children and Young People's Service. The role will require the successful candidates to work in collaboration with the Team Manager to support and mentor a children's social work team, offering development opportunities to less experienced workers. In addition, the candidate will manage a case load of more complex case work within the service.

Key responsibilities:

Support senior colleagues with the prioritisation, allocation, planning and review of work in accordance with policies, procedures and statutory requirements, contributing to effective budget management, ensuring value for public money and the maximisation of service efficiencies.
Contribute to service monitoring and evaluation, undertaking needs analysis, reviewing availability of resources and development of service practice, policies and strategies to maintain high quality services.
Provide supervision, direction and co-ordination of workloads, support and guidance, appraisal and professional development and support social work students on first and final placements; and students from other professional disciplines.
Undertake clear and detailed service user, carer assessments, including community care, safeguarding, mental capacity and best interest assessments, prioritising, allocating and reviewing work in accordance with relevant statutory requirements, minimising risks, with consideration for service user needs and available options.
Manage high complex cases including their planning and review, promoting independence and early intervention as well as the personalisation agenda and self-directed support to minimise risks and improve quality of life for service users and their carers, ensuring targets are met within agreed timescales.
Work with individuals, families, carers and communities to help them make informed decisions, enabling them to clarify their needs, construct value for money care packages and commission a range of appropriate services, as well as respond to their feedback regarding service provision.
Undertake preventative work and risk assessments utilising community resources and services such as reablement and telehealthcare.
Provide professional advice, guidance and information to a range of internal and external stakeholders as required, giving evidence in court and working with multi-disciplinary teams in relation to complex cases.
Manage complex transitions cases and chair multi-disciplinary team meetings as appropriate to specialised field / local structures.

Qualifications:

Recognised Social Work qualification or equivalent (Nursing, OT or Physiotherapist).
Registration with the appropriate professional body.
Evidence of ongoing CPD, including a minimum of 45 Level 7 Credits from the following: Consolidation, Safeguarding, AMHP, Best Interest Assessor, DOLS and Mental Capacity Act. Higher Specialist Award or Postgraduate Diploma.



Experience:

At least 4 years' post qualification experience within relevant policies, the statutory framework and legislation, including safeguarding.
Experience of working in a health and social work team / care setting e.g. elderly, learning disabilities, mental health and vulnerable people.
Experience of managing complex work situations.
Expertise in producing comprehensive and detailed assessments for more complex cases including safeguarding investigations.
Experience of working in multi-agency settings.



Knowledge:

Demonstrate working knowledge of the Social Work Professional Capabilities Framework and the nine interrelated domains.
In-depth understanding of legislation, policies and procedures relevant to working with adults and children, including an advanced understanding of local and national safeguarding procedures and practice.
Demonstrate in-depth knowledge and understanding of personalisation, care planning, problem solving and analysis.
Working knowledge of the CPD framework and the Practice Educator Framework.
Demonstrate an awareness of the role of information systems for monitoring and evaluating the service.



Skills & Abilities:

Excellent oral and written communication skills including listening, complex report writing, presenting evidence and chairing multi-disciplinary meetings and the ability to communicate effectively with the public using a suitable level of fluency in spoken English, in line with the English Language requirement for public sector workers.
Demonstrate excellent skills in forming effective working relationships with service users and their carers and with a range of internal and external stakeholders, providing advice and guidance and constructive challenge when required.
Ability to work in an anti-oppressive manner particularly in regard to service users, carers and families.
Ability to carry a complex caseload, undertake investigations and enable clear and detailed assessments, supporting the development of appropriate packages of care.
Ability to work both as an individual as well as part of a team, and to both model and facilitate reflective and evidence-informed practice.
Demonstrate a flexible, innovative approach to work, as well as willingness to develop specialist areas of work and critically analyse own personal development.
Able to undertake any travel in connection with the post.
Strong IT skills including Microsoft packages or equivalent (e.g. G Suite).
