Apply now to be considered for this unique and innovative leadership opportunity to help lead the way in developing Advance Choice Documents (ACDs) with South London and Maudsley NHS Trust.

The Advance Choice Document supervisor will provide supervision to directorate ACD facilitators and an administrator. The post holder will support facilitators to work across systems with service users who have previously been detained under the Mental Health Act to co-produce Advance Choice Documents (ACDs) with all the services they use across the Trust. The post holder will provide strategic oversight of the development of co-produced, good, evidence-based practice for production of ACDs in mental health services and leading on delivering implementation strategies for their completion, access and use, including awareness raising and staff training.

The post holder will work across inpatient, crisis and community mental health teams to provide supervision of the development of advance choice documents and to facilitate this process through awareness raising and training for all stakeholder groups: service users, cares/informal supporters, team staff and other staff whom service users wish to involve in the process such as peer supporters, advocates or support workers.
To provide highly specialist supervision on the planning, co-ordination, delivery and evaluation of high-quality evidence based ACDs across the Trust.

To supervise the planning and implementation of interventions in the event of a relapse, delivered by the facilitator in collaboration with the service user and the relevant clinical services in the development of their ACD.
To provide assurance on the monitoring, evaluation and modification of the suitability of the content of ACDs developed in collaboration between facilitators, service users and their clinical services.

To apply a comprehensive range of communication skills with service users, carers, key supporters, multidisciplinary colleagues and external agencies.
To work as a member of the multi-disciplinary team, including the implementation of core decisions with regard to service users’ care and treatment.

Maintain appropriate, up-to-date, written and electronic records and activity data in accordance with Professional and Trust standards.
To collect and report on outcome required for the ACD implementation, research and PCERF .

To travel (e.g. home visits, community placements, meetings) as appropriate and across the Trust when required.
To be aware of risk relating to aggressive and challenging behaviour amongst the client group and follow trust policies relating to its management.
