Summary

We have an exciting opportunity to join us as Junior Sister/Charge Nurse on our Virtual Ward

Virtual wards allow people to get the care they need at home, safely and conveniently, rather than being in hospital by utilising remote monitoring technology and medical devices to assess their needs, and to ensure that clinical staff provide the care required to maintain them safely at home. This includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital.

Virtual wards are suitable for a range of conditions that can be safely and effectively monitored at home including cardiac conditions, respiratory conditions, frailty, orthopaedic and surgical conditions.

This is your opportunity to help the Trust to deliver this exciting new model of care, and we are looking for individuals who are ready for a new challenge to help us to safely develop and deliver our model.
