Summary
The post holder will partake with the physiological assessment of our service users. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests Assessing service users holistically. Assessing lifestyles exercise and activities, eating and drinking, smoking, alcohol and drug use and giving advice on healthy lifestyles and signposting our service users to other services. The post holder will work alongside the physical health nurse in clinics and community visits but will be participate in team working. The post holder will work with other team members to ensure the team operates a holistic and multi-disciplinary approach The post holder will contribute to the CPA process liaising with the care coordinators in view of contributing towards CPA needs and reviews Staff benefits Pay Enhancements 30% additional for Evenings (8pm onwards) and Saturdays and 60% additional for Sundays and Bank holidays. 27 days annual leave plus bank holidays rising to 29 after 5 years and 33 days after 10 years Excellent pension Cycle to work scheme Salary sacrifice car scheme Wellbeing programme Blue Light Card Discounts
