Summary

Main duties and responsibilities: To ensure that Navigators mobilise timely and effective support to meet the needs of the clients, enabling clients to maintain their independence, make informed decisions, and to help to prevent unplanned hospital admissions. To ensure that all referrals are offered basic information on benefits and to ensure that a referral to Welfare Rights is offered and actioned in a timely way. To assist with implementing scheme documentation and access criteria and an assessment tool to identify level of independence, monitoring tools for reporting. To ensure that Navigators work with members of the public who are referred to the service, visiting them at home or preferred setting as appropriate and maintaining a high standard of service to the client. To ensure that Carers are well supported across the service areas by ensuring the team have access to up to date information on support available to Carers and how to access support. To deal with day-to-day general enquiries ensuring people receive a prompt, professional and reliable service. To take referrals from different sources, including the Single Access Point, and be responsible for providing/collecting appropriate information and/or actioning support from volunteers or providers signposting when and where appropriate, ensuring that the referral is actioned within the 72-hour timeframe as set by the funder. To ensure all scheme activities are accurately recorded to contribute to and demonstrate measurable outcomes, producing monthly and quarterly reports as required by Service Manager, EMT and external funders. Be prepared to travel to meetings, visiting clients and attending meetings as required across Sandwell. To organise and attend marketing events for professionals and members of the public as and when required. To attend multi-disciplinary team meetings and to explore other opportunities to ensure that information on how the service can support clients is communicated, and to adopt a problem-solving approach. Post holder will be expected to undertake training or study as identified for the post, including undertaking Adult Safeguarding training and becoming designated DSL for Adults, working alongside other DSL trained staff.
