As a Support Worker in Intermediate Care Service, you'll play a crucial role in providing essential support and care to individuals in need. The main duties of a Support Worker in Admission Avoidance include providing personal care to patients, delivering and demonstrating prescribed equipment, performing vital observations, ECGs and venepuncture, supporting patients with exercises initiated by physiotherapists and maintaining accurate records.



You will also collaborate with healthcare professionals, communicate effectively with patients and their families, ensure a safe and clean environment, and actively participate in training and development opportunities.



Adaptability, compassion and attention to detail are essential in this role to provide high quality care and support to patients in their homes.
