The Role

As a Care Co-ordinator you will play a pivotal role in ensuring our customers receive the highest quality of care.

You will be the central point of contact for customers, families, and our team of care workers, coordinating and managing care plans to meet everyone’s unique needs.

The purpose of the post is to support the Service Manager and team leaders to ensure that customers’ care and support needs are met, that the service runs smoothly and efficiently and that the service complies with CQC standards of quality and safety.

Key responsibilities:

• To be a leader within the service and empower the team members in everything they do.
• To support customers and care workers by coordinating their care visits and calls.
• To ensure that care workers with the correct skills and experience are matched to deliver excellent care to our customers.
• To effectively prioritise tasks to meet unexpected emergencies and changes in care packages.
• To complete and distribute care team rotas on a weekly basis.
• To talk with customers and their chosen representatives about their care and support considering different communication needs and levels of understanding.

About you

Our ideal Care Co-Ordinator will have:

• Previous experience of working in a care environment, dealing with difficult and complex situations.
• Knowledge of care co-ordination within a care setting is essential.
• An understanding of an Extra Care setting, is desirable

Person Specification
• 5 GCSEs A-C or equivalent including English and Maths desirable
• QCF Level 3 Health and Social Care or willing to work towards, is essential
