Relief Social Care Assessor, Uist & Barra
£16.85 per hour (inclusive of £1.42 Distant Islands Allowance per hour)
As a member of the Assessment and Care Management Team the main purpose of the post is to maximise the independence and safety of adults and older people who have care needs. This will be done by undertaking assessments of need using the appropriate assessment tools and undertaking reviews of care arrangements. You will assess and support service users in need, to develop appropriate care packages to meet the service user / carers preferred outcome within specified resources or by enabling the service user to access community resources and to review care arrangements.
It is essential that you hold a professional Social Care or Health qualification (HNC / SVQ Level 3) and / or the ability to work towards the registration requirements of the SSSC (if required). You will have a good working knowledge of community care legislation, an understanding of community care policy developments and a working knowledge of assessments, risk assessments and care planning. Excellent verbal and written communication skills, the ability to work collaboratively with other agencies / organisations and the ability to work on own initiative are essential for this post. It is essential that you hold a full, unrestricted UK driving licence.
This post is subject to membership of the Protecting Vulnerable Groups (PVG) Scheme in respect of regulated work with children and adults.
Salary for the above post is inclusive of Distant Islands Allowance. Hours are on an as and when basis.
