Summary
The post holder will be employed as part of the Healthy Families Programme and work with the diverse, multi-ethnic, deprived and highly mobile population across County. A key aspect of the role will be developing links and building supportive relationships within primary care, relevant statutory and voluntary agencies. The post holder will be a source of expert knowledge and support for health colleagues and work collaboratively with partner agencies. You will work with children, young people and families within the home environment, school or other identified community setting. A key aspect of the role is to work flexibly to meet the needs of the service as required.
