Summary
The Homeless & HIU Service is an excellent award-winning service (MBE award, two-time Winner of Lammy Award, Regional Winner of Health and Social Care Award for Improving Health and Reducing Inequalities, Winner of GSTT Care Award, Winner of Ab Fab Award) and friendly team delivering services to those experiencing homelessness. The post holder will be predominantly working in non-NHS sites such as hostels, day centres and street nursing when required and will be expected to travel to the Gracefield Gardens office when needed. The Team also offer flexible working opportunities.
We are currently recruiting to a Band 7 Nurse Practitioner (Adult) post primarily working to deliver the Blood Borne Virus screening service in Southwark. This is a senior post and will contribute to the leadership and development of our service with expert contributions relating to Blood Borne Viruses (testing, treating and aetiology of hepatitis and HIV. This post is to provide exceptional high-quality nursing care for those who are risk or homelessness or rough sleeping in Lambeth, Lewisham and Southwark. The new team members will be supported by senior staff, who have many years of experience and knowledge. • Primarily work as a Nurse Practitioner within the drug and alcohol services in Southwark, developing, providing, and evaluating a seamless, comprehensive blood borne virus (BBV) screening service
• Demonstrate expert knowledge and skills in the area of blood borne viruses, homelessness, addictions, and client groups at risk
• Supervise and manage Band 6 nurses on the team
• Manage a caseload of patients within the specialty whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care that is holistic and resourceful. This includes history taking, conducting physical examinations, and requesting agreed diagnostic procedures including taking blood/clinical investigations.
• Provide specialist advice, education, and support to health care professionals and patients
• Establish and maintain effective, supportive relationships with all key stakeholders across the Trust and in the community, including patients, carers, clinicians, and managers
• Act as an advocate for patients within the department
• Provide a leadership role model and demonstrate expert knowledge and high standards of clinical practice.
• Manage own caseload and practice, identifying and maintaining supportive networks for self and other staff members.
• Provide cover for the other Nurses as appropriate
• Maintaining and updating the BBV testing protocols in collaboration with the specialist hospital teams

The Homeless & HIU service has many years of experience of working with marginalized and vulnerable cohorts. The approach is to complete a comprehensive health assessment, catch-up immunisations where needed, refer into main stream services if required and support individuals with managing their chronic conditions while in temporary accommodation. The nurses/HV/AHPs need to have assessment skills that mean they can confidently complete assessments but also respond to any immediate health needs such as acute infections. We have a set of PGDs that are available to treat minor ailments, administer vaccine and other medications. We are a supportive team with an aim that all team members have the necessary skills and support to be able to offer a safe, culturally competent health service to our client groups.
• · Primarily work as a Nurse Practitioner within the drug and alcohol services in Southwark, developing, providing, and evaluating a seamless, comprehensive blood borne virus (BBV) screening service.
• Run clinics both independently and jointly, and ensuring that patients receive the highest standard of clinical care. This clinical work might include direct patient care, health promotion work, or specialist clinics (e.g. vaccination).
• Demonstrate specialist clinical leadership and management in the local areas of health inclusion and BBV screening, as requested by the Service Lead
• Develop and review team initiatives and projects for the client group
• Actively undertake clinical audits and contribute to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues
• Ensure the safety and well-being of services users, always working in a manner that promotes dignity and human rights through the adoption of person-centred care principles
