We have a great opportunity for you to join our Children’s Community Nursing (CCN) Team;
You will be leading holistic assessment, care planning and review of clinical care for children and young people with a range of health needs to optimise their health outcomes.

All our CCN’s take the lead on an area of community clinical practice e.g. IV’s, enteral, palliative care, respiratory, oncology, It is important for you to develop and continually influence good quality practice in the community and with partner organisations.

We recognise it may feel daunting to move into a leadership role in the community setting, where you’ll be expected to lead and work independently.

If you are an experienced Band 5 and have the required clinical skills and leadership experience we would still love to hear from you. We have a successful development programme to support you growing a community career and achieve competencies for the Band 6 role within a year.

The post requires the job holder to have excellent communication skills, time management skills, clinical competence to deliver clinical nursing care independently and respond to unexpected demands and scenarios in a variety of community settings.

Main Duties and Responsibilities:

Co-ordinates the day-to-day provision of the CCN caseload, ensuring patient’s needs are met, new referrals triaged in a timely mannerand workload allocated according to need.

To undertake clinical assessments, implement, deliver, and evaluate nursing care for children and young people within a defined caseload in clinics, patient’s homes, schools or other appropriate community settings.

To deliver skilled clinical nursing care which may include administration of Intravenous therapy, injections, syringe pumps, blood taking via central venous access devices, wound care, nursing support for all types of enteral feeding tubes, tracheostomy care, bowel and bladder devices.

Using a case-co-ordination approach support families to understand and manage their child’s condition safely at home and in collaboration with multi-agency providers to ensure the child’s aspirations and health needs are central to decision making and maximise their opportunities to develop.

To provide patient/parent/carer training and assessment of competence.

To assist in the discharge planning process of a child’s transfer from hospital to home to identify clinical care and support that can be safely delivered in the community.

To act as a signpost for families enabling them to access support from a range of statutory and voluntary organisations.

If you have a non-medical prescribing qualification, you will be expected to practice as a prescriber as part of this role

To ensure that clinical guidelines and electronic patient record systems are evidence based and maintain clinical effectiveness.



Communication

Provides clinical advice for children, young people, their families and professionals working with them to understand and manage their health needs.

To agree and write care plans/reports to support families/education and care staff understand and care for the child’s condition and health needs using a partnership approach.

To work in partnership with a range of health, education and care teams sharing information and reviewing the care plan in response to the child’s aspirations and needs.

To develop communication pathways with hospital colleagues to implement joint clinical care pathways.

Communicates effectively and compassionately with people who may be distressed or angry, e.g., where there is a child with special needs or a life limiting illness.

To input clinical data to electronic patient records to enable data gathering to evidence service activities in addition to statutory clinical record keeping.



Budgetary Responsibility

To use resources responsibly and ensure timely orders for clinical equipment are made to maintain stock control.

To ensure clinical equipment is used and maintained as intended and that patient safety is assured at all times.

To maximise appointment availability to ensure patients are seen in the most appropriate setting for their clinical needs.



People Management

To be responsible for the day to day supervision, organisation and leadership of the team and prioritisation of workload.

Undertake appraisals and regular supervision of team members; provide learning opportunities, support and assessment to achieve personal and professional objectives.

Contribute to the support and supervision of student nurses acting as a student appraiser/supervisor where appropriate.

Maintain own professional development and requirement to take part in appraisal and KSF process.

To undertake role of line manager for non-registered staff and/or Children’s Community Staff nurses.



Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes.

The post holder must adhere to infection control policies and procedures.

It is a condition of your employment that you are currently registered with your professional body, and it is your responsibility to maintain your professional registration.

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

The post holder must participate in clinical and safeguarding audits as required.

Provide patients and their families /carers with information on standards they should expect from the team.

Participate in Clinical & Safeguarding supervision on a regular basis.

The post holder is required to participate in relevant emergency preparedness process for their team.
