Responsibilities

Actively participating in the patient programme of care and treatment with other members of the
team to provide skilled nursing care to a high standard.
Taking on the role of duty worker in co-ordinating the service as delegated by the Team Leaders.
Working alongside service users to identify their needs and goals utilising the RiO assessment, Trust agreed outcomes measures and the Wellness, Recovery Action Plan.
Key working/care-co-ordinating service users: managing own caseload.
To teach/coach and supervise junior staff within the CMHT as required.
Organising regular individual review meetings and ensuring that all treatment/care programmes
achieve the agreed goals.
To positively promote independent living of service users within the community by promoting
independent daily living within the local community ('living ordinary lives') through regular and
practical support to individuals and their carers.
To help service users gain access to resources including benefits and welfare rights and provide
information to enable individuals to access appropriate health and social care (including healthy
living and promoting healthy living).
To attend service user's outpatients/appointments and ward rounds as required.
To plan and implement programmes of therapy, activity and/or leisure and support with daily
living by working with a Recovery approach e.g., social inclusion.
Administering medication/complying with the nursing responsibilities under the Medicines Act
1968; the Misuse of Drugs Act 1971; the local area of Drugs Policy, and the Trust Care of Drugs
Policy.
Ensuring high standards of communication with service users, relatives, and staff from all
agencies.
Expanding knowledge base/skills in therapies identified to meet service users' needs e.g.,
cognitive behavioural therapy, motivational interviewing, and phlebotomy etc.
