Summary
We are delighted to be looking to appoint a Band 6Diabetes Developmental Specialist Nurse. Working within a well-established community diabetes specialist service which provides high level virtual, face to face and group support for people with Diabetes within the community and working closely as part of a specialist multi-disciplinary (MDT) community service.
The post holder will work autonomously with supervision,actively involved in research and development at local and national level to deliver evidence-based care, education including DESMOND, and information to people with diabetes and their families referred to the Community Diabetes Specialist Service in Brent.
The post holder will demonstrate and understanding and commitment to providing all people with Diabetes referred to the service, the help they need to identify what is important to them, what they would like to achieve and how they plan to achieve it.
The Post holder will demonstrate that they have the attitudes and behaviours required to support people to make and sustain lifestyle changes, provide specialist clinics, structured education programs for adults with diabetes and training programs for health care professions.
The successful candidate will need to demonstrate excellent communication skills, a practical, forward-thinking approach to service development and evaluation and the ability to work as part of a team.
