Summary


Due to exciting developments within our service, REACT (Rapid Emergency Assessment and Care Team) are looking to recruit a motivated and dynamic Occupational Therapist to join our team into a newly devised Band 5 -6 Development post!

The successful candidate will be supported through a comprehensive competency framework specific to Allied Health Professionals, which will include a Preceptorship period if this has not been completed previously.

This role would be ideal for a HCPC registered Occupational Therapist looking to develop their clinical and leadership skills within the Emergency and Acute Medicine environment, including Acute Frailty.

Our service covers 7 days and extended hours (08:30-19:30 Monday-Friday), enabling a variety of flexible working patterns.

Due to the nature of the post we welcome applications from Occupational Therapists currently practising, returning to practice or those that are newly qualified and have an interest in Emergency Medicine.

Working alongside an experienced, knowledgeable and supportive team the successful candidate will have the opportunity to develop complex assessment and decision-making skills with a focus on admission avoidance and rapid discharge. In addition to this, professional and leadership skill development will be a focus through completion of comprehensive competency documents to evidence the transition from Band 5 to Band 6.

Clinical:
1. Be professionally and legally responsible & accountable for all aspects of own work
2. Carry a clinical caseload consisting of patients with a varied range of conditions and health & social needs, with support from senior therapists for those with more complex needs
1. Complete holistic and comprehensive assessments in order to determine appropriate treatment plans & discharge pathways, ensuring that decisions are patient centred
2. Work collaboratively with patients, their relatives/carers and external health/social/voluntary services in order to avoid unnecessary hospital admissions and minimise length of stay
1. Work as an autonomous member of the multidisciplinary team, contributing to team discussions and influencing decisions about patient care and discharge plans
2. Carry out therapeutic intervention using a broad range of modalities and clinical reasoning to allow selection of the most appropriate technique with the view to improve function and reduce deconditioning
3. Evaluate therapeutic/discharge plans and modify as required in response to changing patient need
4. Identify and prescribe appropriate equipment essential to support a timely and safe discharge, including appropriate risk assessment as required
5. Complete accurate and timely documentation in accordance with REACT’s Documentation policy
6. Provide specialist reports on patient function, need and professional recommendations as required
