Are you an experienced nurse with a passion for cardiac care and rehabilitation? Do you thrive in a dynamic environment where you can make a difference in the lives of patients? If so, we have an exciting opportunity for you to join our small team and contribute to the delivery of our cardiac rehabilitation exercise and education program.

As a Registered Nurse, you will play a vital role in our community-based cardiac rehabilitation program. Currently, we provide five exercise classes in the community, three assessment clinics per week, and an online education program. Your expertise and compassion will be instrumental in helping patients recover and improve their cardiovascular health.









SUMMARY OF POST

To assist in ensuring a comprehensive and seamless service is provided for patients with cardiac conditions referred from primary and secondary care.

In collaboration with the cardiac rehabilitation lead, co-ordinate an effective cardiac rehabilitation service. This aims to encourage patient independence following a cardiac diagnosis offering an individualised menu of cardiac rehabilitation including exercise, education, medication management and psychological support.

In collaboration with the cardiac rehabilitation lead, and consultant cardiologists, review the service needs as appropriate.

KEY RESPONSIBILITIES

PRACTICE
• To monitor systems for the effective identification of eligible and suitable patients and invite them to attend a cardiac rehabilitation programme.
• For each patient, develop an individualised programme of specialist cardiac rehabilitation advice to support their recovery. This may involve family and carers.
• To independently run a phase II clinic for cardiac patients referred to the service, via face to face or by telephone/video link clinic
• To participate in the development and running of the cardiac rehabilitation services for phases I, II, III and IV.
• To work as part of a team in the outpatient exercise and education component of the cardiac rehabilitation programme.
• To ensure effective two way communication with members of the multidisciplinary team involved in cardiac services.
• To provide specialist input to the Peterborough area patient led support group.
• Work independently to manage own caseload of patients.

EDUCATION AND TRAINING
• Deliver the education /training programme to patients and their carers as appropriate.
• To be actively involved in the education and training of the multidisciplinary team to develop their professional skills in accordance with the needs of this client group.
• To be actively involved in the education of student nurses in all aspects of cardiac rehabilitation.
• Maintain own continuous professional development through self directed learning, attendance of relevant education programmes or professional conferences.

RESEARCH, DEVELOPMENT AND AUDIT
• Participate in local and national audits.
• Maintain a database to facilitate audits.
• Foster an environment where practice is research /evidence based audit to ensure its effectiveness.
• Identify and disseminate relevant research.
• Participate in relevant research projects.

MANAGERIAL
• Manage own projects as part of working within cardiac rehabilitation service to further develop an evidence based programme.
• Assist in the day to day running of the cardiac rehabilitation services in the absence of the cardiac rehabilitation lead.

HEALTH AND SAFETY

Ensure compliance with the Trust’s health and safety and risk management policies and procedures; establish and maintain safe systems of work within the department or area you work. You have a duty to maintain the health and safety and welfare of staff, patients or other third parties visiting your department or general areas of the Trust.

INFECTION CONTROL

It is the responsibility of all staff to unsure that they understand and follow the infection control policies, procedures and best practice applicable within the Trust. In particular:
• Observe all infection control policies and practices relevant to the post.
• Act as a role model to others regarding infection control best practice.
• Ensure that they keep up to date and attend all relevant training relating to infection control issues applicable to their post.
• Ensure that patients, visitors, contractors are aware of and follow infection control best practice where applicable.
• Report non compliance or concerns regarding infection control issues/best practice to their line manager
