Summary

The post holder will need to use a high level of communication skill to ensure messages are understood where there may be barriers to understanding and gain agreement and co-operations. The post holder will be required to meet with patients and carers within service and ensure clear understanding of case management and complex programmes of care on a continuous basis, discuss potential transfers to alternative units with patients when necessary. This is an opportunity for a candidate who has ambition to work in a unique environment, using their assessments skills to support some of the most vulnerable individuals in the community. You will work in close partnership with Local Authorities, NHS colleagues and care providers, producing excellent reports and ensuring the provision of high quality care to individuals. An exciting opportunity has arisen within the Mental Health and Vulnerable People Team (MHVP) for a band 6 Clinician/Social Worker. The successful candidate will join North Yorkshire Place as part of Humber and North Yorkshire ICB and work alongside key stakeholders such as North Yorkshire County Council, NHSE, Transforming care and Providers in order to work collaboratively with Colleagues and share knowledge and skills. The MHVP team work alongside the NHS Continuing Healthcare team and have an excellent collaborative working relationship and are involved in joint training and development of the service, alongside other stake holders. Our overall aims are to ensure innovative, responsive and effective clinical commissioning and that our population has access to the highest quality healthcare providing the best patient care within available resources. We achieve this by working in partnership with our member practices and localities, with patients and the public, with local councils and other healthcare providers.
