About our Team

We are committed to providing adult care and integration services that put the person at the centre of our work. We are passionate about following the ‘Home First’ pathway and work closely with our hospital colleagues to achieve this.

We cover a range of areas such as intermediate care and Athorpe (non-acute bed bases) as well as community reviews and supporting with longer term reviews. We work collaboratively with health partners in the Community Hub.

There are some new and exciting opportunities for workers joining the Integrated Discharge Team to support and cover several areas including:

Covering A&E and AMU to support prevention of hospital admissions – leading to safe supportive discharges through our joint working hospital avoidance.

Working collaborative with colleagues within RDaSH Mental Health – supporting patients under the Mental Health Act with an organic mental health diagnosis to promote safe and timely discharge from the Woodlands

Working closely with the neuro and stroke pathway.

Successful candidates will join supportive and established team which is comprised of nursing and social work professionals based in a newly built and modern office using interactive technology. As part of this role, you will be expected to support others in the team, including newly qualified Social Workers and Assessment and Review Coordinators to develop their skills and experience.
