Summary


The Health Improvement Worker will work as part of the Local Mental Health Team and wider PCN to deliver physical healthcare interventions to patients with Severe Mental Illness

The post holder will have a specific remit to support achievement of the physical health standards for people with SMI by engaging service users in routine physical health checks and support the person to work towards personal physical health goals in order to improve their physical and mental health. This involves supporting people with on the GP Serious Mental Illness (SMI) register living in the locality to have their cardiometabolic risk factors measured.

Physical health checks of patients

To be responsible for the assessment and monitoring of the physical health needs of service users on the SMI register

To ensure that service users receive regular physical health checks on an annual basis and from an identified caseload provided by e-HealthScope

To interpret the results of physical health tests and act upon instances when follow up / intervention is required.

To develop care plans in collaboration with service users related to improving their physical health

To devise and develop and signpost to supportive physical health interventions accessible to service users.

The Physical Healthcare Practitioner will be accountable and report to the LMHT team manager

The expectation is to provide physical healthcare interventions for a caseload of patients.

The role will involve physical healthcare screening, monitoring, the identification of physical healthcare needs, implementation of physical health interventions.

The role will involve working across different bases in Nottinghamshire
