Are you a 1st Level nurse with 1 years’ post registration and Community Nursing experience.

In conjunction with Salford University we are pleased to offer a seconded development opportunity to the Apprenticeship PgDip Specialist Practice Programme (District Nursing pathway).

This is an exciting opportunity, with the programme delivered through the apprenticeship route. This will enable students to achieve the knowledge, skills and clinical competencies to practice safely and effectively in the area of Specialist Practitioner Qualification/ District Nursing.

You will need to demonstrate knowledge of current issues within the NHS, Community Nursing, and their potential influence on District Nursing, together with a commitment to study at level 7



The post holder is required to complete the District Nurse PGDip Apprenticeship programme through education and work-based learning successfully. They are to meet all the gateway requirements when advised by their employer and undertake the end assessment EPA. The apprentice will work in an allocated District Nursing team conducive to learning that will meet their core skills, knowledge, and behaviour relevant to the programme specialism meeting the needs of contemporary healthcare for individuals, communities, and populations.The apprentice will work alongside a Practice Supervisor (Band 6 CSP) who has responsibility for a designated caseload of patients within the neighbourhood. The Practice
assessor will confirm student achievement of proficiencies and programme outcomes for practice learning, informed by feedback sought and received from practice supervisors and work in partnership with the nominated academic assessor to evaluate and recommend the student for progression for each part of the programme

Main Duties and Responsibilities


CLINICAL AND OPERATIONAL RESPONSIBILITES
• Undertake responsibility for a delegated caseload under the supervision of the District Nursing Sister and in the absence of the District Nursing Sister, carrying out initial assessment of care needs and the provision and evaluation of individual patient programmes of care including prescribing equipment and the discharge of
patients from the caseload
• Assume delegated responsibility for holistic assessment and the planning, implementation, monitoring, and evaluation of programmes of care/ care packages, contributing to complex assessment/ care packages.
• To provide high quality palliative care to patients and their families, as required.
• Work with other health care colleagues, voluntary and statutory organisations to ensure that the holistic needs of patients are met.
• Develop and implement risk assessments and management strategies taking people’s views, choices and responsibilities into account, whilst promoting safeguarding of individuals, carers and staff.
• Actively contribute to public health initiatives including health needs assessment and projects to meet health outcomes.
• Promote health education and healthy lifestyles within the community, including use of motivational interviewing and negotiation skills where appropriate.
• Act as an advocate to facilitate the views and choices expressed by patients and their carers’
• Refer people to other practitioners when needs and risks are beyond one’s own scope of practice or require longer term support

Communication

Responsibility for Patient Care
• Under the direction / supervision of a qualified registered District Nurse manage a caseload of patients in a variety of community settings using a person centred approach.
• Work collaboratively with the patient and the multidisciplinary team. To improve anticipatory care, self-management, facilitate timely discharges and reduce avoidable hospital admissions and enable care to be delivered closer to home.
• In conjunction with the District Nurse manage and co-ordinate programmes of care, for individuals with complex acute and long-term conditions, striving to make the patient journey seamless between mental and physical health care, hospital and community services and between primary and community care.
• Autonomously assess the complex health needs of people, families and other informal carers’ to formulate care plans, anticipatory care and care packages.
• Use evidence based physical and clinical assessments of individuals with complex health care needs to make accurate diagnostic decisions, including prescribing

Responsibility for Human Resources and Leadership
• In conjunction with the District Nurse lead and support the clinical team to provide community nursing interventions in a range of settings.
• In conjunction with / under supervision of the District Nurse manage and deliver district nursing in line with relevant legal, regulatory and governance requirements, policies and ethical frameworks, including mandatory reporting duties.
• Apply the theories, techniques and models of leadership, management, team working and how these are utilised across professional boundaries in health and social care.
• Ensure that equality and diversity issues are recognised, addressed in accordance with legislation, policies and procedures
