Summary


To work as an integral member of the Hounslow Adult Learning Disability Health Team (HALDHT) whilst having responsibility for a designated caseload (with support/steer from higher banded nurses). The post holder will be expected to positively represent the needs and rights of people with learning disabilities, tackling inequalities in health, and improving access to identified services in the statutory and 3rdsectors


• To be able to fully demonstrate and practice the Trust’s values in all aspects of the role when delivering care, working with families, carers and colleagues
• Carrying out holistic assessments and providing clinical advice and support
• As a specialist nurse, work with individuals to address the health inequalites experienced by people with learning disabilites and to hold an active caseload
• Working in partnership with generic health services, service users and carers to support the implementation of Health Facilitators and Health Action Planning, supporting practice nurses, GPs and other primary and secondary care services (including mental health)
• At all times promoting inclusion for people with learning disabilities in all health initiatives across NW London ICB (Hounslow area) and the Local Authority area
• To work in partnership with Service Users, Families and Carer’s

An exciting opportunity has arisen for a dynamic, innovative, and enthusiastic community staff nurse/RN to join our multidisciplinary team for adults with learning disabilities. This post is particularly suited to a newly/recently qualified RNLD. However, if you’re a Registered Nurse from a different part of the nursing register and you have a passion or a special interest in wanting to work with people with learning disabilities, their families and carers then I’d really like to have a conversation with you. We will also consider applications from 3rdstudent nurses nearing the end of their training and are about to register with the NMC.
