Summary


We are seeking a highly motivated and experienced nurse (Flow Nurse) to join our fantastic Care of the Elderly Team at Queen’s Hospital (QH).

You will play a pivotal role in shaping a compassionate, equitable, and transparent working environment aligned with our Trust values. Collaboratively, you will ensure the delivery of high-quality care through effective leadership, mentorship, and partnership with MDT working.

You will benefit from guidance of the Director of Nursing, Head of Nursing and work closely with the ward teams, Clinical Site Practitioners and ward managers. Working closely with the FAME Clinical Group at QH, you will:

- Support patients’ flow and discharges from the wards.

- Working closely with ward managers, senior nursing teams, MDT and discharge team.

- Contribute to the development and implementation of innovative clinical practices and education.

- Play a strategic role in enhancing service delivery and efficiency.

• To provide expert support/ leadership and education in Geriatrics Division.
• To deputise in the absence of the Matron to support the Division.
• Actively participate in meetings and trust meetings to ensure that professional issues are fully represented.
• Collaboratively communicating and working with clinical team both within and outside of the trust to ensure appropriate management of patient information and coordination of patient pathway.
• The post holder will assist in the data entry and development maintenance of database to capture patient information (Discharge Pathway & MCA), monitoring information by proactively identifying and taking action to enable patients to prioritise, admitted/discharged as per individual management plan.
• Monitor and ensure the most effective uses of bed within the Geriatrics Division, follow patients progresses on a daily basis and prevent any discharge delay wherever possible, escalate any delays in patient flow meeting or Flow Lead/divisional management teams.
• To lead the development, improvement, management, and promotion of the service, ensuring the delivery of high quality, cost effective care.
• To lead on the implementation and monitoring of adherence to relevant clinical guidelines.
• To educate and share expertise with the multidisciplinary teams.
• To act as a resource in the care of patients with specialist care needs, ensuring an optimum level of service to patients and their carer.

We provide care for the residents of three diverse London boroughs. Most of our 7,800 permanent staff live in Barking and Dagenham, Havering and Redbridge and the majority are from black, Asian and minority ethnic groups. We also provide healthcare services to people in south west Essex, and specialist neurosciences services to the whole of the county.

Our services include all the major specialties of large acute hospitals and we operate from two main sites - King George Hospital in Goodmayes and Queen’s Hospital in Romford. We have two of the busiest emergency departments in London – more than 300,000 people visited our A&Es in 2023. We also provide outpatient services at Brentwood Community Hospital, Barking Community Hospital, Loxford Polyclinic, and Harold Wood Polyclinic.

We’re pleased to be leading the way inreducing the time our patientswait to get the treatment they need. The Elective Surgical Hub at King George Hospital is one of eight to beaccredited as part of a national scheme; the Care Quality Commission has raised the ratings for urgent and emergency care at Queen’s and King George hospitals; and data released by NHS England showed that the Trust was the most improved in 2023 for reducing waits for emergency care.

We are particularly proud of our regional Neurosciences Centre; Radiotherapy Centre; Hyper Acute Stroke Unit; and dedicated breast care service at King George Hospital. We’re also part of theNorth East London Cancer Alliance.
