Summary
To provide an effective and efficient administrative and clerical service to all members of the team with an emphasis on Social Care functions in Mental Health. This would involve the taking of minutes in a range of clinical meetings, including Safeguarding planning meetings, case conferences and multi-agency professionals meetings.
The post holder would be expected to carry out safeguarding actions on the health and local authority databases. This would include collating and recording safeguarding data, and maintaining a minimum data set of demographic details.
The post holder will be expected to work flexibly within the administration team and support colleagues when required to.
The post holder would be expected to maintain up to date review schedule of all care packages including S117 aftercare, Care Act and Continuing Healthcare funded packages of care. The post holder will assist in the organizing and scheduling of care package reviews. The post holder will assist in the gathering of information relating to S117 aftercare status. The post holder will support Teams to implement a review schedule of all existing packages of care.
The post holder would work closely with the Social Care Lead, Senior Social Worker and Operational Managers to provide an efficient and responsive administrative provision to the service.
The post requires the ability to work on own initiative and to liaise effectively with a broad range of professions and to maintain client confidentiality.
