Summary
Purpose:
To encourage and support adults with learning difficulties and/ or autism on the journey to independence. To provide care in the persons own home over a 24hr period 365 days a year. To be part of a circle of support that will involve maintaining relationships and provide opportunities to make new connections. To provide support that will allow the person to be part of their community if they choose to do so and to enhance the likelihood of reaching long and short-term goals.
Responsibilities:
To support an individual over a 24hr period as per rota
To support the person with any personal care that is identified within the care plan.
Assist with daily living activities, meal preparation, and cleaning
Help individuals develop and maintain independent living skills
Support individuals in participating in community activities and social events
Document and report any changes in behaviour or health status to the appropriate supervisor
Collaborate with a team of professionals to ensure the well-being of the individual
Requirements
- SVQ 2 Health and Social Care
- Previous experience working with individuals with disabilities or special needs is preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient demeanour
- Flexibility to work various shifts, including evenings, weekends, and holidays
Rolling rota:
Shifts 8 am-8 am
Week 1- Tuesday and Saturday
Week 2- Monday and Thursday
Week 3- Friday and Sunday
Week 4- Wednesday
