We are looking for a anti-coagulant Biomedical Science Advanced Practitioner or a Lead Nurse, to lead and support our anti-coagulant team who can demonstrate highly developed specialist knowledge and skills in the management of patients requiring anti-coagulant therapy.

You will line manage the staff within the team. The Biomedical Scientist Advanced Practitioner will formulate holistic care plans to manage and support the patient on oral anticoagulant therapy and will respond to actual and potential health problems appropriately.

The role ensures the provision of excellent care with compassion working with patients requiring anticoagulant therapy, developing a culture of patient self-management, allowing patients to develop responsibility for managing their own condition. Providing clinical expertise, specialist advice and support across service boundaries.


The anti-coagulant BMS Advanced Practitioner /Lead Nurse works as a key worker to the patients providing information, enabling communication and in coordinating care.

Duties to included but not be limited to:
• Responsible for the undertaking of Nurse/BMS-led clinics
providing assessment and treatment of patients
• Monitoring and documenting their progress and modifying
drug treatment in accordance with British Committee for
Standards in Haematology (BCSH)
• Take responsibility for the planning and organisation of the
anti-coagulant clinics within the Trust
• Provide an in-patient pre-discharge service in order to
assess new patients, identify their psychological,
physiological and emotional needs in relation to their
condition and proposed anticoagulant therapy
• Assesses people’s information needs and provides
relevant information to meet those needs at defined points
in the patient pathway
• To provide specialist clinical knowledge to patients,
families and carers following referral to the anti-coagulant service
