Summary


Occupational Therapist - Band 6 Full-Time 37.5 hours per week

Coventry Memory Assessment and Community Dementia Service

Based in Coventry

Do you want to be part of a supportive, forward thinking, positive and dynamic memory assessment and community dementia team?

We have a vacancy for a Occupational Therapist in our Discharge to Assess Service. We are based at Longford Primary Care Centre, Longford Road, Coventry. The post-holder will see patients in Discharge to assess bedded units based in residential homes in Coventry and in their own homes.

You will receive regular support and supervision from the Management and Leadership team. Ensuring that staff are well supported is important in our team. We also welcome opportunities for staff to develop and this will be discussed throughout your time with the team. We encourage staff to utilise their own initiative in order to aid their own development.






• To assess service users using a variety of assessment tools, specialised observational and communication skills.
• To involve and empower both the individual and/or their carer to make decisions about their care and treatment now and for the future
• To provide a learning environment for staff and students.

Registered Occupational Therapist with post registration experience

Experience of working with and conducting assessments with individuals with a diagnosis of dementia or recent experience of working with older adults and conducting comprehensive assessments.

Responsibility for the day-to-day delivery of occupational therapy to individual clients and their families and carers via a range of evidence-based interventions.

To manage and organise competing and conflicting priorities and manage change in complex situations.

To be a resource of clinical knowledge and competence in the service area contributing to staff and service development.

To be a motivated and dynamic individual who has a commitment to the positive development of this service whose aim is to support people to live well with dementia.
