Summary
Assessment and formulation of treatment plans and risk management plans. Crisis intervention and sign posting to relevant agencies, such as, CMHT's, CRHT, mental health inpatient referrals, third sector agencies. Working collaboratively and participation in Multi Disciplinary Team meeting's. Awareness of physical health conditions. Good understanding of the Mental health and mental Capacity Acts and how these are applied in an acute hospital setting. Providing support and supervision to junior members of the team. Providing, support and acting as a point of contact to the acute hospital. Participation in internal audits.
