Summary

Nursing Management Responsibilities Line manage the practice nursing team inclusive of practice nurses & health care assistants Undertake one-to-ones, appraisals, regular practice nurse and HCA meetings, clinical education, and other practice-wide learning events and meetings. Support the development of others to maximise staff potential in the team. Identify the potential for service developments, risk and deficits and inform the HBAF, by making recommendations based on specialist knowledge. To be responsible for ensuring maximum optimisation of use of appointments across the nursing team. Develop and review audit protocols and process for the practice, effectively utilising the audit cycle and to oversee those within the team. Act as a clinical lead in the delivery of practice nursing services to patients, ensuring that the needs of the patient are the priority. Develop and implement new nursing services in line with the practice strategic direction. Lead the nursing team in the planning and implementation of local guidelines, protocols, and standards, as well as local projects or initiatives. Work with the HBAF to ensure we monitor practice performance of QOF and Locally Commissioned Services regularly to achieve high standards of patient care. Ensure health and safety and infection control are high priorities, supported by other members of the team. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) To support the clinical team with all safeguarding matters in accordance with local and national policies. Clinical Nursing Responsibilities Provide primary care nursing support, patient interventions and treatment within the practice. Work within your scope of practice and be responsible for providing patient-centred, culturally appropriate, and individual holistic care to patients. Follow-up of patient test results and communication with patients where appropriate. Health screening, [MN1]asthma review, diabetic review, cervical screening. On-going well-health education and advice, including counselling to improve health and prevent disease. Use of recall and reminder systems and referral, as appropriate, to national screening programmes. Family planning services, provision of contraceptive advice and sexual health services. Delivery of immunisation programmes. To administer medication for patients in accordance with PGDs and own scope of practice On-going care and support for people with chronic and terminal conditions. Health promotion to the practices enrolled population, linking to public health programmes at a national, regional and local level and utilising such programmes to target specific populations To maintain accurate clinical records in conjunction with existing legislation Other Attend regular practice meetings. Take responsibility for own development, learning, and performance, including organising clinical supervision where appropriate and acting as a positive role model. To support the team in dealing with clinical emergencies
