Summary


Come and join our friendly Community Nutrition and Dietetic Team delivering Diabetes care to the Nottinghamshire area.

Our team of experienced Diabetes Educators, Dietitians, Dietetic Assistant Practitioner, and Clerical staff work together to deliver a wide range of nutrition and dietetic services in the community. This includes structured education programmes to people with Diabetes. Our commitment to professional and service development ensures that we deliver evidence-based patient-centred care to improve patient outcomes and experience.

This role is to be part of the award-winning Nottinghamshire Team of Diabetes Educators. Our Educators find delivering the DESMOND course incredibly enjoyable, fulfilling, and rewarding. You will work as part of the team within the Nutrition and Dietetic department, under the direction of a registered healthcare professional educator and Dietetic Team Leader, to provide high quality evidence-based diabetes structured education for people with Type 2 Diabetes. Providing content information isn’t the only focus for the course; you will support individuals with type 2 diabetes within the group to reflect on their current thoughts and feelings about their diabetes and its impact on their lives, actively consider the health risks they face and work towards setting realistic behavioural goals through action planning.





• Provide Structured Diabetes Education (DESMOND) to service users throughout Nottinghamshire both face to face and virtually.

• You will co-deliver the DESMOND Patient Structured Education Programme to groups of up to 20 adults with Type 2 Diabetes and their carers’, family or significant others as set out in the programme manual in a consistent co-ordinated manner. This may be delivered virtually and/or at a variety of community venues.
• You may be asked to deliver other patient education sessions as appropriate and identified by team leader
• You will maintain record keeping in line with professional and trust guidance
• You will become an accredited DESMOND Educator and be responsible for your own clinical professional development and DESMOND accreditation
• You will participate in mandatory training as required by the Trust
• You will actively participate in a regular clinical and managerial supervision to support your ongoing professional development
• You will have the opportunity to contribute to audits and quality improvement projects within the team
