This is an interesting and varied vacancy for an innovative and dynamic Band 6 Practitioner to work in the Lancaster and Morecambe Rapid Intervention and Treatment Team (RITT). The team is part of the wider older adult community mental health team supporting those within the Lancaster and Morecambe catchment areas.

The successful candidate will have sound clinical and leadership skills, be enthusiastic about the role and its development and be driven to provide excellent mental health services to the local older adult population.

Ideally, applicants should have experience of working with older adults with mental health needs and the ability to lead on assessment and provide treatment plans. They should also have excellent communication skills.

The core functions of the service are: The Community Mental Health Team, Memory Assessment Service, Home Treatment and Care Home Liaison. The post holder would be primarily based within the Home Treatment team, but would be expected to provide proficient mental health practitioner input across all service functions in-line with profession and support cover for a 7-day service. You will be required to work a flexible shift pattern within the hours of 8am – 8pm.

The role will involve working within a MDT to support older adult mental health service users in the community to remain at home and prevent hospital admission. Please see the job description for further details about the role and expectations.

In addition to core professional skills, the post holder will be expected to undertake a range of other generic clinical duties to support the team function and be responsive to the needs of patients, their family and carers.

To undertake mental health assessments including Risk assessments within a multi-disciplinary assessment team. Working closely with the Consultant Psychiatrist, thus ensuring that there is a multidisciplinary team approach.

To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.

To provide a fast, responsive service to assess individuals needs for access into mental health services.

Providing a communication structure between GP’s, NHS talking therapies and other services in LSCFT, non-statuary service, as a means of ensuring the highest possible standards of patient care.

To provide mentoring and clinical supervision to other staff and trainees on placement.
