Urgent Care Youth Workers will provide a hospital-based intervention service that supports young people following presentation at hospital and prior to discharge with an injury caused by violence or exploitation or who demonstrate violent or concerning behaviour whilst in hospital. The role will also involve some work with patients in the community. The post holder will be part of a team working alongside hospital staff to proactively identify vulnerable young people. Youth Workers will help these young people to access and engage the support they need to prevent any potential escalation of violence and reduce the risk of repeat victimisation or exploitation.
Young people will be offered trauma informed, emotional and practical support in a non-judgmental way to ‘navigate’ their way away from violence, criminal activity, or other harms (whether they are a victim, perpetrator, or both). This could include signposting and referring onto statutory or community organisations to help the young person with mental health needs, housing, employment, education and training, substance misuse, relationships and supporting young people to make informed decisions about their next steps.
The post holder will:
• Provide a service for young people who present at Emergency Departments with injuries from violent incidents or who present with violent behaviour to navigate them into relevant support.
• Engage and support young people, adopting strength-based approaches and the principles of co-production and social inclusion.
• Work as part of a Multidisciplinary Team to support risk assessment and deliver a high standard of support with positive outcomes.
• Provide advice to young people, carers, families and professionals across the whole system; this may involve ‘signposting’ and liaison.
• Promote positive partnerships, collaborative working and establish and maintain close and effective relationships with all stakeholders.
• Maintain close links with other services to facilitate appropriate interventions and escalation.
• Build a relationship of trust with young people and understand the importance of empowering them.
• Identify barriers to support of the young person and gaps in services.
• Work in partnership with the Here for You Manager and Complex Needs Team Manager and to access support each week to manage personal wellbeing.
