Summary

Role Purpose:
- Increase the number of people with severe mental illness (SMI) receiving annual physical health checks
- Raise awareness about the importance of these checks among patients, carers, GP's, and third sector providers
- Reduce barriers to accessing annual physical health checks for those with SMI
- Coordinate personalised care planning, engagement, and psycho-social support for NICE-recommended interventions
Key Responsibilities:
- Promote awareness of SMI annual physical health checks
- Coordinate marketing & engagement activities
- Receive referrals and assist patients in arranging appointments
- Identify and address barriers to access
- Co-produce personalised access support plans
- Collaborate with stakeholders to improve systems and protocols
- Monitor, evaluate, and provide insights for commissioners
