Summary



We are looking for a dynamic, organised and highly motivated individual to join our cardiac pharmacy team on our Hammersmith hospital site. The team forms part of a world-class tertiary referral cardiac centre led by specialists with academic interests and leaders in their field. We use cutting-edge clinical innovations to treat and prevent heart conditions and offer a full range of investigations, treatments and management for cardiac conditions. It is a challenging and varied service, from the cut and thrust of our heart assessment centre, through to our cardiothoracic surgery unit, recently HSJ award winning heart failure team, and specialist pulmonary hypertension and lipid services.

The role will be focussed primarily on assisting in the development of and providing a pharmacy service to the cardiac wards (including the Heart Assessment Centre, cardiac day and cath labs), delivering education and training and also gain experience in directorate work including management of high cost medicines, new drugs applications, guideline review and drug expenditure reporting.

The post holder must be self-motivated and organised with excellent communication and interpersonal skills. They will need to be able to develop effective working relationships with a wide range of professionals, and will need good IT skills and be able to work with many different IT programs & databases.

Applicants should be a pharmacist registered with the General Pharmaceutical Council. You should have completed up to 18 months of a postgraduate clinical pharmacy diploma and working towards full diploma or equivalent experiential learning.





· To assist in the development of a high quality patient focused clinical pharmacy service to patients on the cardiology and cardiothoracic surgery wards, including the heart assessment centre, cardiac day ward and cath labs.

· To assist the senior lead pharmacist in the management and development of medicines management initiatives to cardiovascular services

· To assist the senior lead pharmacist in the management and development of band 6 pharmacists and pre –registration pharmacists when on rotation to cover cardiovascular wards.

· To provide cardiology ward services (involving early working, where appropriate) and to cardiothoracic surgery wards when required.

· To provide clinical pharmacy services, department and ward based, in line with Trust procedures and national legislation.

· To ensure the safe, clinically effective & cost efficient use of medicines within each rotation.

· To improve the quality of pharmaceutical care to patients.

· To gain experience of the funding and monitoring of high cost drugs
