Summary


We are currently looking for a motivated and experienced Registered Nurse (Adult and/or Mental Health) who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for individuals being assessed for Continuing Healthcare (CHC) funding or who are in receipt of CHC funding.

This is a key position within a changing service and provides a good opportunity for the successful candidate to gain experience and learn new skills.

The candidate should have knowledge of the NHS Framework for Continuing Healthcare.

The successful candidate will provide specialist assessments and reviews for adults and older people presenting with long-term care needs who may be eligible for NHS Continuing Healthcare, NHS-funded nursing care and jointly funded packages of care in line with the NHS Framework for Continuing Healthcare.

Experience of undertaking assessments, and working with patients, families and carers is essential. The Personalised Healthcare Team are responsible for the assessment and review of people who reside in nursing care homes and the clinical commissioning of packages of care for people with continuing healthcare needs who reside in their own homes.

Currently the post is following a hybrid model with a mixture of office working, home working and face to face patient visits. The office is based in Stoke on Trent and the role will involve some travelling in and outside of the Staffordshire area in order to assess or review people placed in out of county placements.

Co-ordinating assessments for NHS Funded Nursing Care and Continuing Health Care.

To provide clinical commissioning oversight to applications and assessments concerning joint funded packages of care.

Seek authorisation of eligibility recommendations via Clinical Lead for CHC or locality lead as necessary within financial limitations.

To participate in the CSU Ratification process and also Appeals and retrospective appeals process – attending meetings as required and completing the required reports in line with local process.

On-going review of service users eligible for CHC, NHS funded care in accordance with National Framework for NHS Funded Care, and Joint funded care packages of care.

Undertake urgent reviews of service users, where quality and safety concerns have been identified and report in a timely manner.

To prepare/complete appropriate documentation/tools to enable the level of NHS funding to be determined within the parameters of the National Framework for Continuing Health Care and Funded Nursing Care and Joint Funding arrangements locally.
