Summary

Utilise experiential knowledge and clinical skills to ensure effective application and interpretation of the Mental Health Act and Who Pays Guidance for s117 aftercare NHS funding. Work collaboratively with other health and social care colleagues across the system to assist in the determination of an individuals care needs that sits above Local Authority Funding and Primary and Secondary Healthcare Services. Educates, informs, and advises patients/carers/relatives to support informed choice. Maintain accurate and contemporaneous records and facilitate, support, maintain and improve record keeping systems. Support the Team Leaders to undertake Quality Surveillances such as completion of Safe and Wellbeing Reviews. The post holder will require good verbal and written communication skills to communicate with patients, their families and other professionals. The production of accurate, detailed and comprehensive reports is essential. Communicates sensitive and complex medical/nursing information effectively, accurately, and concisely to colleagues and professionals of other agencies. Provides education to nursing and social care staff of other agencies within both the private and public sectors with regards to NHS responsibilities under s117 Aftercare Participates in training and induction of new staff members to the team. .To undertake training as identified appropriate to the role and identified through the appraisal process. To maintain the confidential nature of clinical and management information within the requirements of the Data Protection Act, Caldicott guidelines and ICB Policies with regard to confidentiality. Actively works towards developing and maintaining effective working relationships both within and out of the ICB. Fosters and maintains strong links with all Services across the ICB, developing key contact points within care and corporate services. Develops and maintains good relationships with patients, carers, colleagues and professionals from other agencies in both the public and private sectors.
