An exciting opportunity has arisen to be part of a unique national oncology service, hosted by Chelsea and Westminster Hospitals.


The UK Cancer and Transition Service(UCATS)provides integrated care for gender diverse patients who have experienced cancer.UCATS isoperatedby the Department of Oncology at Chelsea and Westminster Hospital and hosted within theTransPlusGender Identity Pilot at 56 Dean Street.


In our second year of operation, we are looking for an enthusiastic, passionate and motivate clinical nurse specialist to join our multidisciplinary team. This role will involve direct support to patients, as well as clinician education and promotion of the service. In collaboration with the administrator and the rest otherteam members, youwill also willhelpdevelop the service throughpatient and public involvement activities.


As a team player they will need to be able todemonstrateexcellent listening and communication skills, be organised, adaptable and flexible, show empathy and concern for others.

Main duties of the job


The post holder will contribute to the delivery of high quality, safe and compassionate healthcare to patients by providing an administrative service focused on the patient and their individual needs.





Thepatient will work together with the UCATS team of doctors and a specialist nurse to respond to:


-Providetimelyresponse to referralsand gather further clinical information frompateintsand their care teams


-Respond to clinical enquiries and escalate to other members of the UCATS team when needed


-Attend UCATSvirtual clinics (1stand 3rdMondays of the month)


-Provide direct support to patients via the telephone pre- and post-appointment.


-Support development ofour clinical registry


-Assist with development of and collection of experience and outcome measuresbysupporting patientand public involvementmeetings,anddeveloping written and electronic questionnaires.


-Promote the service throughlocal,nationaland international presentations

Roles and responsibilities

• Support the delivery of ahigh qualityhealthcare service ensuring that all patients receive a high standard of service delivery
• Handle queries from patients/relatives confidentially and sensitively; referadvancedclinical related queries toappropriate staff
• Interact with patients and relatives in distress in a sensitive and empathetic manner
• Provide an excellent standard of service to patients at all times
• Ensure promptresponse to clinical queries from patients and healthcare teams
• Assistwith development written and electronic questionnaires.
• Liaise with patients and clinical teams to gatherappropriate clinicalinformation atreferraland during follow-up


• Clinic bookings and notification of patients and their clinicians
• Follow-up on referrals
• Providedirect patient supportviatelephone
• Attend UCATSvirtualclinics
• Co-develop patient outcome and experience measures
• Assistin developing the clinical registry
• Assistin developing guidelines and educational materials
• Promote the service through local,nationaland international presentations to clinicians, patient groups and other stakeholders.
• Assisttheadministratorwith responsibility for theday to daymanagementof the service
• Set personal and professional objectives
• Seek education in the field and keepup-to-datewith relevant services to signpost patients to
• Activelyparticipatein audit, research and the evaluationof the role and the service
• Promote good practice and adopta holistic approachto patient care at all times
