Summary

Applications are invited for the position of Specialist Fellow in Major Trauma and Adult Critical Care for a 12-month period in August 2024.

This post would ideally suit candidates with anaesthetic/ICM experience who are aiming to advance their CV and develop their ICM & Acute Care Skills to a higher level within a Major Trauma ICU.

The successful candidate would spend 50% of time developing a specialist interest in Major Trauma with opportunities for research, education, quality improvement and development of clinical expertise/skills related to major trauma.

Suitable applicants will be offered opportunity to become faculty on the NUH Trauma, Anaesthesia, Resuscitation and Simulated Care Course and supported in applying for a place on the European Trauma Course.

Clinical Responsibilities will include (not exhaustive):

· Perform daily reviews on ICU patients and present findings on the MDT ward round, with a focus on our major trauma patient population within adult critical care.

· In conjunction with the consultant support the clinical decision making of junior medical and nursing staff as the senior member of the critical care team

· Review critically unwell patients outside of critical care.

· Admit, stabilise and formulate an initial management plan for critically unwell adults.

· Competently initiate ventilation, use vasopressors, insert CVC and Arterial Lines and be able to transfer the critically unwell patient.

· Attend trauma calls and with appropriate direct supervision and training:

o perform high quality primary surveys of low, medium, and high injury severity triage patients.

o Resuscitate the unstable major trauma patient including providing immediate haemostatic measures, site wide bore central access, initiate, and manage a massive transfusion.

o Coordinate the initial management of a patient with a high injury severity score as they transition from the emergency department to time critical imaging and then to intervention for haemostasis / damage control surgery in theatre or interventional radiology.

NUH consists of the Queens Medical Centre Campus and the Nottingham City Hospital Campus. Duties will be based at both campuses.

On the QMC campus the critical care area is divided into a 29-bedded intensive care (level 3 beds) and a 20-bedded level 2 HDU. The critical care area at the City campus has 17 beds that are used flexibly for level 2 and 3 care. Between the three units we see approximately 4500 admissions per year.

We are fortunate to have a large and skilled MDT to support the medical team in delivery of critical care to Nottingham and the wider catchment area.

Nottingham is a diverse and vibrant city having a busy and culturally exciting centre, two universities and yet proximity to picturesque villages, the Peak District, Sherwood Forest and Rutland Water. In 2020 Nottingham was described by The Times as…

“one of our favourite cities: fun, funky and affordable, with a swagger to match Manchester's, but on a much more manageable scale. It's a thriving shopping hub with a cracking arts scene and has one of the best public-transport networks in the country.”

The post is open to all applicants who meet the essential criteria as detailed in the person specification below. It is open to all doctors who hold Full/limited registration with the GMC and successfully completed foundation year 2 training and a further 2 years in acute specialities (of which 12 months should be in anaesthesia) who wish to gain further experience in the management of critically unwell major trauma patients and enhance their CV for application to specialist training or consultant posts.

Critical Care at NUH admits over 150 level 3 (intubated and ventilated) critically ill major trauma patients a year. We work closely with the major trauma, surgical and interventional radiology teams to deliver a high-quality care to a patient cohort with severe injuries. We have a well-integrated major trauma service delivered by two tiers of major trauma consultant: Trauma Team Leaders from a range of clinical backgrounds (EM / ICU / Anaesthesia / Surgery) as well as dedicated Trauma Surgeons. We are one of the busiest major trauma centres in England based on numbers of: trauma calls, trauma laparotomies and trauma thoracotomies per year.

This post is whole time and is covered by the Medical and Dental Staff Terms and Conditions of Service. Doctors applying for this post from overseas should note that prior NHS experience is a prerequisite and we would recommend applying for one of our Trust Grade posts in the first instance.

The post is EWTD compliant.

The Salary for this post will be on the MT04 or MT05 scale depending on experience plus an out of hours supplement.
