Summary


An excellent opportunity has arisen for motivated, dedicated individuals to join a well established and hardworking team on the Waverley Rehabilitation Ward at the Fryatt Hospital on a permanent basis.

We are looking for enthusiastic and dynamic people to support the rehabilitation and reablement of patients with the aim of returning them to maximum independent living, within a stimulating environment, actively promoting self-management on all activities of daily living and working closely with other members of the multidisciplinary team.

Waverley Rehabilitation Ward is a 24-bed community rehabilitation ward which includes 11 stroke commissioned beds. We promote patient involvement and empower patients and family members to provide input into their clinical care and decisions.

We respect that each individual has the right to autonomy and independence and endeavour to promote that independence as able.

You will be working with a large team of therapists within the MDT supporting in-patient rehabilitation, therefore you must be flexible with the ability to work as part of a team and also independently. You must be hard working, rehab focussed and an excellent communicator. If you feel you have these skills, we’d love to hear from you.

You must be able to work a range of shifts including long days and nights, weekends and bank holidays.

The role of the assistant practitioner is to assist patients with their hygiene needs, toileting, nutritional intake, supporting and encouraging patients with their mobility under the guidance of the therapy team, and monitoring and recording vital signs, whilst maintaining their dignity and respecting their individuality.

You will actively contribute to the effectiveness of team working in the nursing and rehabilitation of patients within the service and participate in MDT meetings and case reviews as required.

You will provide rehab focussed nursing or rehabilitation care to patients.

You will carry out treatment plans, under the guidance of the relevant clinical specialist in nursing, rehabilitation or therapy.

You will undertake training, to develop and enhance an understanding of the role of the Assistant Practitioner
