Summary

1.1 Responsible for the assessment of the care needs of children and adolescents, and develops programmes of care in relation to the needs of the child and family suffering from childhood constipation 1.2 Responsible for the implementation of programmes of nursing care and evaluation of their effectiveness, ensuring that the care plan provides a continuous record of progress, response to treatment, rehabilitation and nursing care. 1.3 Take responsibility for a defined caseload of childhood constipation patients within the inpatient/outpatient setting under the supervision of the Childhood Constipation Nurse Specialist 1.4 Work in partnership with children and their families in order to support and encourage involvement in their care, thus supporting family centred care 1.5 Take the lead in supporting parents, carers, siblings and other members of the childs support network as appropriate 1.6 Responsible for, and takes the lead in setting and maintaining optimum standards of nursing care, ensuring that nursing guidelines and policies are adhered to within the inpatient/outpatient setting in relation to childhood constipation 1.7 Maintain effective communication with multidisciplinary team members, other hospital nursing and medical staff, general practitioners, community nursing staff and statutory and non-statutory organisations that may be involved with the care of the child and family 1.8 Attend appropriate meetings to include case conferences, multi-disciplinary meetings and ward rounds 1.9 Run Nurse Led Clinics for children, parents and other health care professionals, ensuring compliance to guidelines set out in the NICE guidelines CG99 Constipation in children and young people: diagnosis and management 1.10 Be aware of the spiritual, psychological and emotional needs of the child and their family and endeavours to meet these at all times 1.11 Ensure seamless transition of care from the hospital to home setting 1.12 Act as a source of specialised knowledge and clinical expertise in relation to childhood constipation, to ensure care delivered to a child and family is appropriate, optimum and evidence based. 1.13 Advise on the supply and safe use of clinical equipment to children and their families within the hospital and community setting 1.14 Responsible for, and co-ordinates the custody of all drugs by checking, ordering and administering them in line with Trust policy both in the hospital and community setting 1.15 Responsible for the location of emergency and resuscitation equipment and is aware of procedures both in the hospital and outpatient setting
