Summary

MAIN DUTIES AND RESPONSIBILITIES OF THE POST A) eConsult Administration Team Member Working within our eConsult Administration Team, the post holder will be responsible for: Accessing eConsultations and filing them within the patients EMIS records. Navigating the patient eConsultation through Sentiers to: o Identify and prioritise the patient need for an appointment. o Identify the relevant Community/Practice Healthcare Professional most appropriate to deal with the patient needs. Referrals to community healthcare professionals i.e. community pharmacies Sending links to self-help and wellbeing support information via text message or email Identifying the need for prior nursing appointments i.e. bp, blood test, ECG, to ensure that clinicians have all the information they require to enable them to consult effectively. Co-ordinating foreign language interpreters, when required, to facilitate clinical appointments. Co-ordinating sign language interpreters, when required, to facilitate clinical appointments. Co-ordinating chaperones, when required, to facilitate clinical appointments. Notifying patients of their appointment date/time via email, text, or telephone. Ensuring appointments have been allocated the appropriate slot types ie. face to face, telephone, urgent on the day or routine. Working with the team members to manage all relevant tasks, emails and telephone calls Directing all administrative queries received via eConsult to the relevant teams. Directing all nursing appointment requests to the Patient Assistant Team. The team will receive the support of an eConsult/Nurse Support GP/Urgent Care Team member who will work with them to triage patient concerns when the Sentiers navigation tool does not provide clear guidance. OTHER REQUIREMENTS OF THE POST B) Professional Duties and Essential Characteristics To always ensure a high standard of practice and care by: Ensuring accurate notes of all patient-related work is recorded in the patients medical records. Maintaining excellent communication, customer service and IT skills at all times. Flexibility to provide cover during periods of sickness and holidays. Attending and participation in practice meetings as required. Maintaining continued education by attendance at courses and study days as deemed useful by the Practice, or necessary for professional development. Working co-operatively with the practice teams.
