We are working with a local NHS Trust who are seeking a Band 6 Community Mental Health Practitioner to support their Early Intervention in Psychosis Service in Walthamstow.
Band 6 Community Mental Health Practitioner
Location: Community based in Walthamstow
Rate of pay: £27 p/h Umbrella
Working Hours: Monday-Friday, 9am-5pm
Contract: 3 month initial contract with the view of extension
Key Responsibilities:
• Conducting thorough assessments and screenings of individuals referred to the EIP team for early signs of psychosis or related mental health issues.
• Providing comprehensive case management for clients, including coordinating care, developing individualised care plans, and monitoring progress towards goals.
• Responding to crisis situations and providing immediate support and intervention as needed. Collaborating with emergency services and other healthcare professionals to ensure client safety.
• Delivering psychoeducation to clients and their families about psychosis, its symptoms, causes, and treatment options. Providing information on coping strategies, relapse prevention, and accessing community resources.
To be eligible for consideration, it is essential that you are a Registered Mental Health Nurse with a valid NMC Pin, with EIP experience (essential).
