Summary

Key Responsibilities Regular attendance of the monthly Saturday Dementia Café supporting attendees from the local community in addition to known patients and families/carers Provide individualised care and prioritise needs in line with available resources and considering local and national policy/guidelines to achieve the best possible outcomes for patients and their families/carers. Ensure that patients dignity, needs, wishes and culture are always promoted and respected and that equality and diversity are priorities of care. Undertake comprehensive patient cantered assessments, to enable patients and families to make informed decisions and agree a plan of care based on clear goals intended to bring about effective, measurable outcomes. With guidance from senior colleagues refer to primary and secondary care for relevant diagnostic investigations. Use a high level of clinical enquiry to interpret results or to raise concerns. With support from senior team colleagues/ medical team or GP review medication regimes and seek to optimize patient concordance and reduce polypharmacy. Proactively oversee anticipatory prescribing to ensure all necessary medication is in place to achieve management of patient symptoms towards end of life in their preferred place of death. Work in partnership with other members of the multidisciplinary team and services to help ensure patients achieve their preferred place of care, utilising end of life tools such as the Gold Standard Framework, Advance Care Plans and RESPECT documents. Promote the work of the organization in all settings, encouraging collaborative working and the referral of every appropriate patient.
