Summary
Purpose of the Job:
To provide person centered care to individuals in their own homes so you must be prepared to travel between care calls.
You will engage with service users to build trusting relationships. Your role will include visiting service users, completing tasks and care needs identified within the care plan.
To work closely with other health and social care professionals and agencies to provide a coordinated service which meets the identified needs of the individuals.
