Summary

Duties and Responsibilities: Recognise and work within own competence and professional code of conduct as regulated by the appropriate professional body Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Prioritise, organise, and manage own workload in a manner that maintains and promotes quality Deliver care according to local and national guidelines Obtain and document consent for any treatment undertaken and keep the patient at the centre of his/her own care, ensuring full participation in decision making Assess, diagnose and treat patients with a range of acute illness and injury Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing the services Work within all relevant policies and procedural guidelines e.g., infection control, safeguarding, chaperoning and risk management Cooperate with the Clinical Governance team when investigating complaints and incidents Engage with Clinical Audit and respond and reflect to learn lessons from feedback. Clinical practice: Autonomously assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis and those with long term conditions in line with national and local policies and practice/ network guidelines Diagnose and manage acute conditions, integrating both drug- and non-drug-based treatment methods into a management plan Prescribe medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Completing an NMP annual declaration form each year for FCMS role. Work with patients in order to support concordance with and adherence to prescribed treatments, provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills Communicates highly sensitive condition related information to patients, relatives and carers Clinically examine and assess patients needs from a psychological and physiological perspective and plan clinical care accordingly Inform the Clinical Manager / Service Coordinator of any untoward incidents e.g. falls, verbal aggression, criminal damage, to then be logged onto the incident reporting system Inform The Clinical Manager/ Safeguarding Lead and Service Coordinator of any safeguarding concerns. The clinician should refer to social services as needed and then ensure this is logged on the incident reporting system. Contribute to infection control measures by adhering to infection control policies/procedures Educational: To follow safe working practices and to comply at all times with local Safety Policies and Procedures To ensure confidentiality on all matters relating to patients and information obtained during the course of employment and not to release such information to anyone other than those acting in an official capacity as instructed Maintain good relationships with other members of the staff team and stakeholders across all disciplines Contribute to the identification of own training needs and participate in relevant in-service training To attend staff meetings as required To keep up to date with clinical guidance e.g. NICE and follow best practice This Job Description will be periodically reviewed in light of developing work requirements. The individual in post will be expected to contribute towards that revision. The Clinical Manager may also ask for other duties to be carried out to the benefit of the organisation. Where necessary relevant training in the operation of new or unfamiliar equipment, software or procedures will be provided or arranged. The post holder will be expected to attend any necessary courses that provide relevant training. This is to ensure there is no question of it creating a conflict of interest with your NHS duties. You must also therefore seek your manager's approval before taking on any such other work or outside interest at any time after entering employment. The role is subject to a DBS check which will require three forms of valid ID to be produced and verified.
