Summary


We are currently looking for an enthusiastic, highly motivated Occupational Therapist in the newly developing Integrated Community Stroke Service within Sherwood Forest Hospitals NHS Foundation Trust.

The current Stroke Early Supported Discharge team were established in 2009 and now has the exciting opportunity from March/April 2023 to expand and develop the service to be able to provide more access to specialist stroke community-based rehabilitation within the localities it covers. This high-profile community-based service covers Mansfield, Ashfield, Newark and Sherwood and receives referrals for patients following an acute stroke.

The service aims to facilitate a smooth and timely transition from hospital to home optimising stroke recovery, function and independence for the initial 6 months post discharge home from hospital.

The team has a strong ethos of patient centred care and high clinical standards. We are looking for an Occupational Therapist that has a keen interest in stroke care and rehabilitation and that is keen for a new challenge and would like opportunities for self and service development.
